I attest that all information on this application is true. I authorize the release of the student applicant's academic records to the UNM Gallup TRIO/SSS program. The information in these records is used only to asses the student applicant's eligibility, and the need for services, determine student applicant's educational progress, evaluate program effectiveness and fulfill reporting requirements.

I also understand that if selected to participate in the SSS program, I will:

  • participate in tutoring and mentoring sessions
  • attend at least two (2) TRIO/SSS workshops per semester
  • attend at least one (1) TRIO/SSS event per semester
  • meet with TRIO/SSS academic advisor per semester

I will participate in the UNM Gallup TRIO/SSS services which are designed to help me reach my educational goals for degree attainment.
